I confirm that my student is currently in the grade I listed above. I understand that misrepresenting my child's current grade can result in their removal from camp without a refund.

I understand that this is a private youth summer camp and particiption requires my child to operate in a group setting. Individual student supports are not available.

I grant permission for the above-named participant to engage in all activities of the above-named program. I, on behalf of my child, voluntarily assume all risks, and hazards to such participation including transportation to and from Sycamore High School. I hereby release, indemnify and hold harmless Sycamore Summer Theatre LLC, the facility owner Sycamore Community Schools and employees of the above for any injury, damage or loss suffered during the program.

I authorize Sycamore Summer Theatre staff to employ medical assistance for my child in the event he/she suffers an illness or accident while participating in the program. I agree that Sycamore Summer Theatre staff may act independently of me should action be deemed necessary for the safety and well-being of my child.
